It is mandatory to have a physical before a player can begin to practice. Turn these forms into the team mom/parent. They must be kept in a binder at practices and games.
-Please turn in proof of player physical
-Photocopy of player birth certificate
-Copy of their last report card dated June 2018 at equipment pickup day.

PLEASE NOTE: Most birth certificates contain the Social Security Numbers of the parents. Make sure to safeguard this information when you are making a copy of your birth certificates.


The below deposits should have been made at the kick-off meeting in June. If you were a late sign-up you may bring it to equipment hand-out along with all of the waiver forms.

Volunteering Deposit $100 - Per child:
Every family is required to volunteer 3 times per season per child participating.
As a volunteer organization volunteering is mandatory for us to be able to run home games for the kids.

1 child = 2 volunteer times
2 children= 4 volunteer times
3 children = 6 volunteer times
and so on....

Sign-up sheets will be emailed out.


Equipment Deposit $100 - Per child:
An equipment deposit needs to be made on equipment pickup day.

All deposits will be returned on equipment return day we do NOT cash the checks. If you have not met your volunteering commitment or did not turn in equipment your check will be considered a donation to the HPYFA.
